5th GRADE DANCE FORMS

Anchor 1

Please print and fill out the 5th Grade Dance Emergency Contact Form before the first dance your 5th grader attends. This allows us to contact you in case of an emergency, etc.

Every child must leave the dance with an adult. We trust that the adult the child leaves with is their adult and is on their Emergency Contact Form. This allows parents to provide carpool transportation.
